Durable Remission in BRCA2-Mutated, PD-L1–Overexpressing Pancreatic Cancer with Multimodal Precision Therapy: A Case Report

This case report describes a 60-year-old man with pancreatic ductal adenocarcinoma (PDAC) and a pathogenic germline BRCA2 mutation who received modified FOLFIRINOX plus losartan with stereotactic body radiation therapy (SBRT) and nivolumab prior to distal pancreatectomy and splenectomy. Surveillance imaging later detected a solitary liver metastasis with PD-L1 combined positive score (CPS) = 100, which was treated with percutaneous microwave ablation, after which the patient began maintenance therapy with olaparib and pembrolizumab. The patient remains in clinical remission more than five years after detection of the liver metastasis, with no evidence of disease on serial MRI and PET/CT imaging.
